HEALTH NoTES
HEALTH NoTES is a new tool to facilitate physician-patient communication to help patients better manage their health. HEALTH NoTES identify key concepts that reinforce physician messages. HEALTH NoTES are booklets written at a level that almost all patients can understand. The NoTES include photographs of real people to help patients understand what they need to know and do.
How can you use HEALTH NoTES?
- During the office visit to guide physician-patient communication
- After the office visit to reinforce what the physician said about managing their health
- With patients' families to help with adherence

The first HEALTH NoTE is on Adult Vaccines. It is a quick guide to help adults understand what vaccines they and their family members may need. The following vaccines are included:
- Seasonal Flu
- Tdap
- Pneumococcal
- Zoster
- Meningococcal
- Hepatitis A
- Hepatitis B
- HPV
